The Transition Activation Worker Program provides a source of funding to support individuals up to 18 years of age with a serious Mental Health illness as they transition back into their community.
The Transition Activation Worker Program supports children and youth by providing:

Short term funding for a trained worker to support the individual in the community upon discharge from hospital/residential treatment.

Flexibility to allow scheduling of interactions that meet the child/youth’s needs.

Monitoring to ensure that the treatment setting provides the clinical supervision.

A close working relationship with Mental Health service providers to ensure opportunities for success in reaching the predetermined goals.
How can I make a referral?

Hospital or residential treatment facility must complete the application for funding on behalf of the client on an ongoing basis.

Applications are reviewed as received.

Decisions are made based on criteria and availability of funding and are communicated to the service provider and family.
