What Is Coordination Services for Children and Youth (CSCY)?

Coordinated Services for Children and Youth program works with agencies and community partners across Durham Region to support children or youth up to 18 years of age who have very complex social, emotional, developmental, and/or physical needs that are beyond what the local service system can support. An Intensive Service Coordinator provides information, consultation, and community case conferencing to ensure eligible children and youth receive coordinated support to meet their unique needs.

How Coordination Services for Children and Youth (CSCY) Can Help Families

Intensive Service Coordinators work closely with healthcare providers, educators, and community organizations to create a plan that brings everyone together. The program makes sure that all resources are explored and used effectively.

If local services aren’t enough, this program helps your family connect to regional and provincial resources, including Ministry-funded programs such as the Child and Parent Resource Institute (CPRI) for eligible children/youth. It can also help you to access services from outside your community if they are needed.

CSCY provides consultation to both families and service providers, helping them understand their child or youth’s needs. Providing consultation to service providers supports continuity of care for families by eliminating the need to establish a new relationship with a different service coordinator. The CSCY Coordinator plays a key role in helping everyone involved make informed decisions and develop a service plan tailored to your child’s unique needs.
