What is Coordinated Service Planning?
Coordinated Service Planning (CSP) is a program that supports families with children or youth (up to 18, or 21 if still in school) who have multiple medical, mental health, behavioural and/or complex needs. This family-centred program connects your family with a Service Planning Coordinator who helps link your child or youth to the right services and create a single, service plan. Service Planning Coordinators work alongside partners such as healthcare providers, educators, and community service organizations to ensure all team members are working toward common goals, so your family can focus on what matters most. - One point of contact: Your family would be assigned a Service Planning Coordinator who navigates services, answers questions, and gathers your story reducing the need to repeat information to multiple providers.
- A single, clear plan: The coordinator works with you to create one service plan based on your child’s unique strengths, needs and goals.
- Coordinated support: Service providers, such as healthcare professionals, educators, and community agencies, work together toward common goals for your child, with the Service Planning Coordinator acting as the central point of contact for both your family and the rest of the team, ensuring everyone is aligned and informed.
- Support with transitions and decision-making: The coordinator assists you during key transitions, such as changing schools, a change in needs or planning for adult services, and helps you make informed decisions.
- Be or have a child, youth or adult up to 18 years of age (21 if you are still in school) who has multiple and/or complex needs.
- Need extra support to navigate the service system.
- Currently living in Durham Region.
