  • Individual-specific questions about your child, family and/or service journey.
  • Referrals to both internal programs at Resources as well as external programs in the community.
  • Families
  • Service providers (with family consent for person specific inquiries)
  1. Contact Us: You can reach out to us directly with any questions or requests, or complete the form below. Service providers, with permission from the family, can also make referrals on behalf of a family.
  2. Response Time: An Access Coordinator will contact you within 4 business days. Depending on your needs, we may offer immediate assistance and/or resources or schedule a short intake meeting to better understand how we can help. This meeting can take place virtually, or in person at a location that is convenient for you.
  3. Brief Service Support: For families requiring short-term help, we provide brief services to address immediate needs and guide you through available resources. This includes:•  Providing information about and referrals to appropriate community services and supports;

    •  Offering support at meetings (for example a school or service provider meeting);

    •  Assisting with completing and submitting applications for funding or benefits.
