Guardians are encouraged to share copies of reports from R.F.E.C.Y. with other involved agencies or individuals. A signed Consent to Disclose Personal Information is required prior to the sharing of information (verbal or written) with other service providers or individuals. We may collect personal information from other sources if you consent or if the law permits. All staff/volunteers sign an Oath of Confidentiality to ensure that information is not communicated to others without permission.
Information is collected and used to:
- Assess client needs.
- Communicate with families and other care providers.
- Deliver safe and efficient programs and ensure a continuous high quality of service.
- Meet legal requirements.
- Verify accountability with funding bodies and internal processes.
Limited information may be used to provide you with news about programs, services and special events.
The client or family may ask to review their personal information and may seek correction of information that is believed to be inaccurate. The client may withdraw consent to usage or disclosure of information by contacting our office.
