No Wrong Door

In the summer of 2011, Resources for Exceptional Children and Youth - Durham Region was Lead Agency on a community project funded through the Ontario Ministry of Children and Youth Services and endorsed by the Durham Best Start Network.

Through the work of community partners and CMCS Consulting, the No Wrong Door process was created. The purpose of No Wrong Door is to make it easier for families living in Durham Region to connect with the services they want for their children ages birth to 13th birthday. As part of this project, a User Guide explaining the process was created. Decision Charts use the information provided by a caregiver to lead the family to a service provider that may be able to assist them. A Family Information form to gather background information and a Consent to Share form were also created to support this process.

The No Wrong Door process builds on existing community resources. When you have identified a service that may be of assistance to the family, use the following resources to learn more about that agency's services, referral process and to obtain contact information.

À l'été 2011, Ressources pour les enfants et la jeunesse exceptionnels - région de Durham était l’organisme chef d'un projet communautaire financé par le ministère des Services à l’enfance et à la jeunesse de l’Ontario et approuvé par le réseau Meilleur départ de Durham.

Grâce au travail des partenaires communautaires et des CMCS Consulting, le processus Portail universel a été créé. Le but de ce projet était de rapprocher les familles de la région des services dont pourraient avoir besoin leurs enfants âgés de 0 à 12 ans (soit jusqu’au 13e anniversaire de naissance) et, par le fait même, d’améliorer la transition entre les différents organismes de soutien. Dans le cadre de ce projet, un Guide de l'utilisateur explique le processus a été créé. Organigrammes décision utiliser l’information fournies par un fournisseur de soins de conduire la famille à un fournisseur de services qui peuvent être en mesure de les aider. Un formulaire Information sur la famille de recueillir des informations de fond et un Autorisation à transmettre de l’information forme ont également été créés pour soutenir ce processus.
